An office meeting was conducted at 2:30pm on February 19, 2026, with Sacramento North Regional Office via Microsoft Teams. The purpose of this meeting was to discuss concerns regarding an incident that occurred at the facility on February 19, 2026 and to discuss facility's plans for future compliance. The following Licensing staff were present: Licensing Program Managers (LPMs) Lauren Crocker and Laura Munoz, and Licensing Program Analyst (LPA) Michael Hood The following facility representatives were present: Licensee Mark Graham and House Manager Stephanie Harmon-Christensen The following documents are being requested as a result of today's meeting: Proof that the facility hired an Administrator by February 23, 2026 Staff schedule removing Licensee from caregiver duties Licensing discussed steps towards changes of ownership during meeting. Facility representatives will follow up with LPA regarding any questions involving applications or the changes of ownership process. No deficiencies were cited. Exit interview was conducted. Signature on these forms acknowledges receipt of these documents.
