What the inspector wrote

On February 26, 2026, a non-compliance conference was conducted. The purpose of this conference meeting was to address non-compliance at the facility after being issued 16 Type A citations and 4 substantiated complaint allegations since being licensed March 29, 2022. Present in the meeting was CCLD staff, including Acting Regional Manager Troy Ordonez, Licensing Program Managers Lauren Crocker and Laura Munoz, and Licensing Program Analyst Michael Hood, and facility staff, including Licensee Mark Graham. The conference process was explained during this meeting. Issues discussed during this meeting were: · An overview of citations issued · Health and safety of the residents in care · Changes of ownership of the facility · Obtaining a certified administrator The facility has stated that they will do the following to achieve continued and substantial compliance: · House Manager will take over operations of the facility · Facility will ensure sufficient staffing to meet the needs of the residents in care · Facility will obtain a certified administrator Facility was notified that the Department may increase monitoring at the facility and the completing the Non-Compliance Conference does not deprive the Department of its authority to take appropriate formal legal action under the Health and Safety Code if such action is deemed necessary by the Regional Manager. A civil penalty in the amount of $200 was assessed for today's date due to failure to correct a citation regarding obtaining a certified administrator. Exit interview was conducted and a copy of this report was provided. Signature on these forms acknowledges receipt of these documents.
