What the inspector wrote

On 09/18/2025 Licensing Program Analyst (LPA) Cheyenne Ratajczak arrived at the facility unannounced to conduct a Required- 1 Year inspection utilizing the inspection tool. LPA met with Administrator Muamoudou Soumahoro and explained the purpose of the visit. LPA and Administrator conducted a tour of the interior and exterior of the facility. Areas toured include but not limited to resident bedrooms, bathrooms, kitchen, common areas and backyard. While conducting a tour of the backyard LPA observed the ramp/ deck leading to the emergency exit gate was in disrepair. Administrator stated they have the supplies but are waiting on the facility maintenance man. LPA conducted a file review of five (5) resident files. Additionally, LPA conducted a file review of one (1) staff file. LPA observed that the staff who was actively working at the facility has an expired CPR/ first aid. Staff stated that they are signed up for a new class. As a result of today's inspection, deficiencies observed. Please see LIC 809-D. Exit interview conducted and a copy of the report and appeal rights was provided.
