Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a required annual and met with Alex Timis, Administrator, and explained purpose of the inspection. Also present were staff, Sanda Rebeca Timis and Joan Freckleton-Jones. (3) residents were resting in the common area and (3) residents were in their rooms. The facility is licensed for (6) residents - (5) can be bedridden and (1) must be ambulatory. Residents are appropriately placed based on the fire clearance. Currently there is (1) resident on hospice- facility is approved for (4). Annual fees were recently paid. LPA and Administrators toured the interior/exterior of the facility including the common areas, resident bedrooms (6), resident bathrooms (3.5), kitchen, staff rooms, laundry. All resident rooms are private and (5) rooms have an exit door. The facility was clean, in good repair and had sufficient furniture/lighting. Resident bathrooms (3) have a walk-in shower with grab bars, non-skid flooring, and hygiene supplies. There is sufficient 2+day perishable food and 7+day non-perishable supply of food. There is a spare freezer/fridge on site with additional food. Sharps, medications and toxins are secured. The inside temperature was 73*F and hot water measured 106*F in the kitchen. There is a tankless water heater set at 110*F. Fire extinguisher last serviced 1/28/25 (will be serviced 1/22/26). Smoke/monoxide alarms are working. Staff are aware of where utility shut-off valves are. There are no exit doors with blockages. There is a complete First Aid kit and manual. Fire/emergency drills are conducted monthly, at different times of day. (2) resident files and (3) staff files were reviewed. Files were organized, current and complete. Medications were reviewed for (2) residents- orders match medications- electronically mainted. Staff have completed required training and RCFE Administrator certificate is current.#7001261740 (exp 11/4/27). LPA obtained updated copy of liability insurance, LIC308 and LIC500. There were no deficiencies observed . There are no citations issued in this report. Exit interview. Copy of report provided to Administrator.
