On 3/4/26, Licensing Program Analyst (LPA) Talwinder Bains arrived unannounced to conduct a case management inspection following receipt of incident report (LIC624), death report (LIC624A) for a resident ,R1, on 3/3/26. LPA met with staff and explained purpose of inspection. Administrator Sarvejeet Bhandal came after short while and assisted LPA with today's visit. LPA interviewed Administrator and discussed death of resident (R1) in more detail, including resident's recent condition prior to passing. LPA reviewed R1s paperwork, including but not limited to Physician's Report, Appraisal/Needs and Service Plan and Centrally Stored Medication Record . LPA requested R1s file documents to be sent to LPA via email by 3/4/26 by 5PM. At this time, further analysis is needed, and a copy of death certificate is also requested for review. There are no deficiencies being cited during today's inspection. Exit interview. Copy of report left at facility.
