Correction check·April 16, 2025

Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a Proof of Correction visit. LPA met with caregiver and explained the purpose of the visit. LPA spoke with Licensee on the phone. LPA was informed corrections has been made and submitted to LPA. At this time, LPA is requesting the following Plan of Corrections by April 17, 2025. If not received, LPA will return at a later date to cite $100 per day Failure to Correct civil penalties. Licensee will create a drill schedule to be in compliance and provide a copy. Licensee is to conduct an audit of staff files to ensure all caregivers have a first aid certification. Licensee is to provide proof of S1 and S2's first aid. Licensee is to provide LPA a statement of compliance of the following regulation 87307 Personal Accommodations and Services (d) The following space and safety provisions shall apply to all facilities: (6) All outdoor and indoor passageways and stairways shall be kept free of obstruction. No deficiencies cited at this time. Exit interview was conducted and a copy of report was provide to staff.
