What the inspector wrote

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a required annual inspection and with staff, Sharita Ferguson, and stated the reason for today's inspection. Administrator Designee, Georgiana Nath, arrived shortly along with Administrator Floarea Stoica. The facility is licensed for (1) ambulatory, (4) non-ambulatory and (1) bedridden resident and is approved for (4) hospice residents. Currently there is (1) resident on hospice. LPA observed (2) residents present today and was advised a resident under hospice passed recently. LPA and Administrator toured the interior/exterior of the facility including the common areas, (4) private resident bedrooms, (1) shared resident bedroom, (2) resident bathrooms, kitchen, office, and staff room. LPA observed the facility to be clean, in good repair and to have sufficient furniture and lighting throughout. The bathrooms have the necessary grab bars, skid-resistant flooring and hygiene supplies. There is sufficient 2+ day perishable food, including fresh produce, and 7+ day non-perishable food. Sharps/toxins are locked in the kitchen and medications are secured nearby. There is a small locked box for medications in the refrigerator. Hot water measured 111*F in the kitchen and the inside temperature measured 77*F. Fire extinguisher last serviced on 3/12/26. Smoke/monoxide alarms work. Exits have auditory alerts and there are no blockages to any exits. There is access to the front area from both the patio. Annual fees due 5/20/26. (2) resident and (4) staff files were reviewed. Files were organized and contained current documentation. Medications were reviewed for (1) resident- no discrepancies noted. All staff are cleared/associated and have completed required training. The facility reported a change in administrators, and Administrator Designee, Georgiana Nath has been acting as Administrator since May 2025- written request to be submitted to the Department by 3/20/26. Current RCFE Administrator Cert #7031377740 (exp 8/2/27). LPA requested current copy of liability insurance and LIC308, LIC500 by 3/26/26. There were no deficiencies observed. Exit interview. Copy of report provided to the Administrator Designee.
