What the inspector wrote

On October 22, 2025, Regional Manager (RM) Alycia Rayner and Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection utilizing the care tool. RM and LPA met with Staff and explained the purpose of the visit. Administrator arrived to the facility shortly afterwards after the physical plant tour was conducted. LPA, RM and Staff conducted a tour of the interior of the facility to ensure the health and safety of residents in care. Areas toured included but not limited to: residents bedrooms, bathroom, laundry room, office, garage, pantry, kitchen and the common areas. LPA advised Staff that air filters should be changed annually to ensure clean filters. LPA observed temperature to be at a comfortable temperature of 72*. LPA observed the carbon monoxide alarm and fire alarm to be operating. LPA observed medication, sharps and toxins to be locked and secured. LPA observed facility to have two days of perishables and seven days of non-perishables foods. LPA observed a canned good to be expired, LPA was informed residents in care typically do not eat canned vegetables. LPA informed staff to conduct an audit of the pantry to remove all expired cans. File review was conducted for four residents in care, and four personnel records. LPA reminded Administrator that staff are to have active CPR and first aid certificates on file. LPA and Administrator discussed the current balance of the licensing fee. Administrator ensure payment will be made immediately. At this time, deficiency cited. Please see LIC 809-D. Exit interview conducted a copy of report and appeal rights was provided.
