On October 29, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a case management visit regarding a pending application for a change of ownership. LPA met with Executive Director and explained the purpose of the visit. During today's visit, LPA, Executive Director and Health & Wellness Director discussed the new application is pending but to ensure that an intent to change ownership is provided to residents in care, their responsible parties, Long Term Ombudsman and Community Care Licensing. LPA was informed that all documents will be provided timely. LPA informed Executive Director that an office meeting may be held with Licensee to ensure the process of change of ownership goes smoothly, remaining compliance to Title 22. At this time, no concerns was reported to LPA. Exit interview conducted and a copy of the report was provided.
