Pre-licensing visit·April 14, 2025

On 04/14/2025, Licensing Program Analyst (LPA) Cheyenne Ratajczak arrived at the facility announced to conduct a Pre- Licensing visit. LPA met with Administrator, Aleksandr Sheludchenko. This application is a change in ownership (CHOW). This address is currently licensed as Only Love Elderly Care Home, Facility # 345002979 . Administrator holds a current administrator certificate (#6071739740 with expiration date 07/30/2026 ). The facility currently has three (3) residents during today's inspection with one (1) receiving hospice services. Facility has a fire clearance for five (5) bedridden and one (1) ambulatory resident. LPA and Administrator conducted a tour of the facility. Areas toured include but are not limited to: residents bedrooms, which were properly furnished and maintained. LPA toured common bathrooms and observed to be clean and sanitary. Grab bars were present at the toilet and in the shower. The food supply is within compliance, two (2) days perishable and seven (7) days non-perishable. Smoke and carbon monoxide detectors present and functional throughout the facility. Fire extinguisher was last inspected on 10/10/2024. All exits were unobstructed. All toxins, medications, and sharps were locked and stored away. Facility's temperature was 77 degrees Fahrenheit during inspection. Hot water temperature was observed to be 110 degrees Fahrenheit, which is within the required range of 105-120 degree. First aid kit found to be complete. LPA and Administrator completed the inspection tool and Comp III. Pre-Licensing completed and no deficiencies was observed. LPA provided Administrator a copy of LIC311F. LPA informed Administrator facility is not licensed until they are informed by Centralized Application Bureau Analyst with a copy of facility license. Exit interview conducted and a copy of the report and LIC809G was provided.
