What the inspector wrote

On 09/23/2025 Licensing Program Analyst (LPA) Cheyenne Ratajczak arrived at the facility unannounced to conduct a post-licensing inspection. LPA met with Administrator Aleksandr Sheludchenko and explained the purpose of the visit. LPA and Administrator conducted a tour of the interior and exterior of the facility. Areas toured include but not limited to resident bedrooms, bathrooms, kitchen, laundry room, common areas and the backyard. LPA observed bedrooms to be properly furnished, with appropriate bedding and lighting. The bathrooms observed to be in sanitary condition, properly maintained. Facility has two (2) day perishable and seven (7) day non-perishable food supply. LPA observed knives, toxins and medications to be locked and inaccessible to residents. Fire extinguisher was last serviced on 10/10/2024. LPA observed fire detectors and carbon monoxide detectors to be operable. LPA observed required Licensing posters posted throughout the facility. LPA reviewed five (5) resident files and two (2) staff files. A review of staff files indicates that all facility staff has received criminal record clearances and/or are associated to this facility. Staff records reviewed indicated current training completed. No deficiencies being cited during today's inspection. Exit interview conducted and copy of the report and LIC809G was left at the facility.
