Pre-licensing visit·October 22, 2025

Licensing Program Analyst (LPA) Sabrina Calzada arrived announced for a scheduled Pre-Licensing inspection at 10:00 and met with Administrator, Curtis Robinson and Board Member, Arminder Takhar. The location is not currently licensed and there were no residents present. The facility is approved for (6) non-ambulatory residents, (1) of whom may be bedridden in room #3. All other rooms are approved for (1) non-ambulatory resident each. The home is newly remodeled. LPA, Administrator, and the Board Member toured the interior and exterior of the facility including the common areas, (6) private resident bedrooms, (2) resident bathrooms, (2)staff rooms and a bathroom, kitchen, and laundry area. LPA observed the facility to be clean, in good repair and to have sufficient lighting throughout. The bathrooms have the necessary grab bars, skid-resistant flooring, and soap. All (6) client rooms are furnished with the required furniture and have closet space also. LPA observed sufficient dishes, flatware and cooking pans in the kitchen, and some perishable and non-perishable food on hand. Sharps, toxins and medications are locked in/near the kitchen. Additional toxins are secured in the laundry room. Hot water measured 105*F in the kitchen and the inside temperature measured 68*F. Fire extinguishers were last serviced on 1/9/2025, and the smoke/monoxide alarms are working, including any fire doors. There are (2) complete First Aid kits and sufficient linens/towels/blankets and flashlights/night lights, and resident games/activities. There is a large patio area and one unlocked exit gate outside. A patio set with umbrella was purchased and will be brought over this week. There are alarms on each exit door. Residents will use a pendant button that is centrally monitored in the kitchen. Required postings are visible in the common area. Land line to be installed this afternoon. Resident files and staff files are prepared and organized. Comp III was completed during today's inspection. Pre-Licensing is complete and this facility has no deficiencies. LPA to notify the analyst in the applications bureau. Exit interview. Copy of report provided.
