Pre-licensing visit·April 1, 2026

Licensing Program Analyst (LPA) K. Hiratsuka, arrived unannounced to conduct a Pre-Licensing Inspection. LPA met with Executive Director Gregory Klick. LPA toured with Health and Wellness Director (HWD) Tammy Alves, who helped LPA with the inspection. This Pre-Licensing inspection is the result of a Change of Ownership. This facility has a fire clearance for 108 non-ambulatory residents of which 10 may be bedridden. The building has two floors. The building has an assisted living side and a memory care side. The memory care has delayed egress. Several rooms on each floor were toured. The facility has a mix of studio, one bedroom, and four two-bedroom apartments and they all have small kitchenettes. The apartments on the assisted living side have full private bathrooms. Most of the rooms in the memory care unit are connected by a shared full bathroom and some have a full private bathroom. There is a salon on both sides, common areas, and yards. The assisted living side have two laundry rooms the residents may use. The kitchen for the facility is on the assisted living side. Component III orientation was waived. This facility meets regulations. LPA is going to submit this report to the application specialist.
