On 10/24/25, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit met with Administrator. The purpose of today's visit is to discuss the recent admission of R1. Records were reviewed. R1 had been recently diagnosed with a prohibited condition. LPA observed R1 and discussed current care plan with caregiver and Admin. LPA met with and discussed R1's condition with family who were present. R1 is due to have their condition reassessed by home health in the coming days. Admin agreed to submit: an update of R1's status, contact precautions in place, copy of training provided to staff re R1's care/ precautions and home health's care plan. Documents to be submitted to LPA by 10/27/25. As a result of today’s inspection, no deficiencies were noted. Report reviewed. Computer malfuntion. Copy of report emailed to licensee for signature.
