What the inspector wrote

On 11/5/25, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit met with caregiver. LPA spoke with Administrator about the reason for the visit. The purpose of today's visit is to discuss the recent admission of R1 that was first discussed in the visit of 10/24/25. Records were reviewed. R1 had been recently diagnosed with a prohibited condition while hospitalized prior to admission on 10/17/25. At this visit, LPA found R1 is deceased on 11/11/2/25. Death report is pending. R1 did not receive repeat testing prior to admission, nor while a resident, to determine if the course of medications to treat their MRSA had been effective. As a result, R1 was admitted and retained , between 10/17/25-10/28-25, with a prohibited health condition without an exception granted by the department. Licensee had initiated contact precautions and staff training. R1 was allowed to stay in the home as they were admitted to Hospice care on 10/28/25. As a result of today’s inspection, deficiencies were noted. Report reviewed. Copy of report and appeal rights provided.
