Pre-licensing visit·September 3, 2025

On 09/03/2025, Licensing Program Analyst (LPA) Cheyenne Ratajczak arrived at the facility to conduct a Pre-Licensing inspection. LPA met with applicant, Syed Fahad Shah, and explained the purpose of the visit. During today's inspection, LPA and Applicant conducted a tour of the interior and exterior of the facility. LPA observed multiple corrections needed. LPA observed the backyard needs to be maintained, as there is not a clear path to emergency exit gate. LPA observed a build up of items laying on the ground in the backyard, which needs to be cleared. Applicant is to ensure that all medication and medication supplies are locked and inaccessible. Applicant is to ensure common areas are ready for residents. LPA informed Applicant to contact LPA Ratajczak once applicant makes corrections and facility is ready. Exit interview conducted and a copy of the report was provided.
