Pre-licensing visit·March 25, 2026

Licensing Program Analyst K. Hiratsuka, conducted this announced prelicensing visit. LPA toured with Applicant Sunny Wong. This facility has a fire clearance for six non-ambulatory residents. There are two shared and two private resident rooms. Two bedrooms have direct exits to the outside. One resident room is on the left of the facility and three are on the right of the facility. There is a fire door leading to a hallway that has three resident rooms and one full bathroom on the right side. There are two full shared bathrooms with grab bars and nonskid floors. There is a locked laundry room. There are locked areas for medications, cleaning materials, and files. The backyard was inspected. There is a double gate on the left side of the house. There is a kitchen, dining, sitting, and office area. Component III orientation was conducted. This facility meets regulations. LPA is going to submit this report to the application specialist.
