Licensing Program Analyst (LPA) Melana Llopis arrived at the facility unannounced on 08/11/2021 to deliver a letter of immediate exclusion for staff (S1). LPA met with Administrator, Alicia Rist and made her aware of the purpose of the visit. Administrator received the letter and was informed to read the letter in it's entirety. Prior to the visit, LPA completed required COVID-19 testing protocols, and a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms; contacted licensee upon visit and completed a facility risk assessment. LPA ensured they applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: Surgical Mask. Additionally, LPA was screened by staff, Vanessa Jones, upon entering the facility . LPA also conducted interviews with residents (R1 and R2). LPA obtained the contact information for resident (R3)'s representative. Exit interview conducted, copy of report provided.
