What the inspector wrote

Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct an annual required inspection. LPA Valerio met with Administrator Hiyasmin Samala, and explained the purpose of the visit. LPA Valerio and Administrator toured the facility to ensure compliance with Title 22 regulations. LPA Valerio observed two direct care staff and four residents in the living room. Common areas were observed to be fully furnished, clean, and free from odors. Resident bedrooms were observed to be fully furnished, personalized to each individual, and free from debris. Resident bathrooms were observed to be clean, fully stocked with hygiene supplies, and free from odors. The kitchen and pantry was observed to have an adequate food supply along with an emergency supply. Storage areas were observed to be used for extra miscellaneous items. Medications, sharps, and toxins were observed to be locked and inaccessible to residents in care. Emergency exits were free from obstructions. LPA Valerio observed resident The last fire drill was held on 09/13 and 09/14/2025. The facility had a fire extinguisher with a last annual service check on 08/23/2025. No health or safety concerned observed. LPA Valerio reviewed three (3) staff files and two (2) resident files. Staff files were up to date with annual training. Resident files were complete with required documentation. LPA Valerio requested the following documents be sent to LPA Viarella: LIC 500, LIC 308, LIC 610, and copy of surety bond Per California Code of Regulations - Title 22, no deficiencies were observed today. An exit interview was held, and a copy of the report was provided.
