What the inspector wrote

Licensing Program Analyst (LPA) Cassie Yang arrived at the facility to conduct a case management visit regarding an incident report that the Department received. LPA met with staff and explained the purpose of the visit, staff contacted Licensee who arrived to the facility shortly afterwards. The incident report reveals that resident (R1) was sent to the hospital on January 15, 2026 for a follow up on their blood work. R1 is currently in the hospital for drainage removal, with the discontinuation order of insulin. LPA and Staff discussed that R1's health conditions has changed due to lack of eating and health deterioration where hospice services may be beneficial. Licensee informed LPA this matter has not been brought to R1's primary care physician's attention has Licensee has been unavailable. LPA and Licensee discussed the importance of notifying primary car physician and responsible party regarding change of condition. Licensee will update LPA once R1 is discharged from the hospital. Copies of R1's observation notes were obtained. As a result of today's inspection, deficiencies cited. Exit interview and a copy of the report and appeal rights provided.

When changes such as unusual weight gains or losses or deterioration of mental ability or a physical health condition are observed, the licensee shall ensure that such changes are documented and brought to the attention of the resident's physician and the resident's responsible person, if any.This requirement is not met as evidenced by: Based on interviews conducted, Licensee failed to comply to the section cited above as R1 has been declining with lack of appetite for approximately two weeks but change of condition was not brought to the attention of R1's primary care physician, which poses an immediate risk for residents in care.

    87109 Transferability of License(b) The licensee shall notify the licensing agency and all residents receiving services, or their representatives, in writing as soon as possible and in all cases at least thirty (30) days prior to the transfer of the property or business, or at the time that a bona fide offer is made, whichever period is longer, as specified in Health and Safety Code Section 1569.191. This requirement is not met as evidenced by: Based on file review and interview, Licensee failed to comply as Licensee did not issue a 30 day notice to residents in care, repsonsible parties, and licensing agency regarding the transfer of property which poses a potential risk for residents in care.

    Based on file review, the licensee did not comply with the section cited above as R1 has a mental health diagnosis but no documentation of appropriate training on file which poses a potential health, safety or personal rights risk to persons in care.
