What the inspector wrote

Licensing Program Analyst (LPA) Melana Llopis arrived at the facility unannounced on 07/08/2021 to open a complaint the Department received on 07/06/2021. LPA was greeted at the door by staff (S1) Marina Doty who made Administrator aware of LPA's presence. LPA spoke with Administrator over the phone and stated the reason for the visit. Prior to initiating today's inspection, LPA completed required COVID-19 testing protocols, and a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms; contacted licensee and completed a facility risk assessment. LPA ensured she applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: N-95 Mask. Complaint alleged staff yelled at R1, R1 is not being provided a nutritious meal and there are no activities for R1 to engage in. LPA interviewed three (3) of three (3) residents in care. Two (2) of three (3) stated they have not heard yelling in the facility and stated the staff treat them fine. Two (2) of three (3) residents stated the meals are nutritious. Resident (R1) stated when they ask for something different to eat, the staff will prepare something different. When LPA entered R1's room to interview them, R1 was lifting weights. R1 stated they walk about a mile a day with one (1) of the care staff and weight lift daily. R2 stated they enjoy doing arithmetic and the facility provided them supplies and books for their daily math activity. R3 stated they are not able to do much activity because of their physical status. LPA interviewed staff, S1 who was present at the facility. S1 stated they have not heard any yelling at the facility or witnessed any staff yell at residents. S1 provided LPA with facility menu the staff try to follow. LPA reviewed menu and found the menu to show a well-balanced food plan. LPA observed facility refrigerator and food pantry and found an adequate supply of 2 day perishable and 7 day non perishable items. During the interview with S1, S1 stated the residents do not prefer much activities other than watching TV in their rooms. No activity agenda could be provided for review. LPA spoke with Administrator when he arrived at about 3:15PM. Administrator stated they do their best to accommodate each resident. Staff will take residents to the store, to go shopping, to go on walks. The facility has books for residents who enjoy reading. Staff allow resident (R1) to eat when they want. LPA observed R1 enter the kitchen and pour themself a cup of juice. Due to the information above, LPA finds the allegation to be UNFOUNDED, meaning that the allegation is false, could not have happened, and/or is without a reasonable basis. Exit interview conducted. Copy of report provided via email due to a consistency check error while LPA was in the field. The facility was provided a PDF copy of report during time of visit. Administrator is to send LPA a signed copy of this document by 7/10/2021.
