On January 09, 2025, at 1:00 PM, Licensing Program Analyst (LPA) Avelina Martinez made an unannounced visit to this facility to conduct an annual required inspection. LPA Martinez met with Romeo Apuya and explained the purpose of today's visit. LPA inspected the physical plant including but not limited to the kitchen, dining room, resident bedrooms; resident bathrooms, laundry room, activity room, and outside courtyards of the facility to ensure compliance with Title 22 regulations. Administrator holds current certificate and expires on November 01, 2026. The facility is licensed for 6 non-ambulatory residents, and has a hospice waiver for 1 resident. There are currently 4 residents who reside at this facility. There are no residents on hospice. The LPA Martinez toured the facility with Romeo Apuya on January 09, 2026, at 2:30 PM. LPA Martinez reviewed four resident files and four staff files. The files were maintained and current. LPA Martinez reviewed two centrally stored medication and destruction records, and LPA Martinez observed the records to complete and maintained. The facility has a emergency disaster plan, and the last fire drill was conducted on January 01, 2026. The facility has an infection control plan. The facility temperature measured at 71 degrees, and the water temperature measured at 105 degrees. Smoke detectors are in good repair. The facility common areas, bedrooms, and bathroom are furnished and sanitary. The exterior of the facility is clear of debris and sanitary. The facility has an adequate food supply. An exit interview was conducted, and a copy of this report was provided at the end of the visit.
