Based on record review, R3 did not have a pre-admission appraisal completed, which poses/posed a potential health, safety or personal rights risk to persons in care.

First aid training requirements

Based on record review, S2 did not have a current first aid/CPR certification, which poses/posed a potential health, safety or personal rights risk to persons in care.

Provide assistance for residents medical needs

87465(a)(2): “(2) The licensee shall provide assistance in meeting necessary medical and dental needs. This includes transportation which may be limited to the nearest available medical or dental facility which will meet the resident's need. In providing transportation the licensee shall do so directly or make arrangements for this service.” This requirement was not met as evidenced by: Based on interviews, transportation was not provided or arranged for R1 for a medically necessary appointment, which poses an immediate health and safety risk.

Personal assistance and care for required daily activities

87464(f)(4): “(f) Basic services shall at a minimum include: … (4) Personal assistance and care as needed by the resident … as specified in Section 87608, Postural Supports.” This requirement was not met as evidenced by: Based on interviews, R1 was not provided assistance with a medically necessary postural support on at least one occasion, which poses an immediate health and safety risk.

"(a) Each licensee shall furnish to the licensing agency such reports as the Department may require, including, but not limited to, the following:...(B) Any serious injury as determined by the attending physician and occurring while the resident is under facility supervision." This requirement was not met as evidenced by: Based on interviews, the licensee did not submit required incident reports after R1 suffered a genital laceration due to a catheter being removed, nor when R1 was sent to the hospital for a fracture of their arm, which poses a potential health, safety, and/or personal rights risk.

Retain original records for at least three years

"(e) Original records or photographic reproductions shall be retained for a minimum of three (3) years following termination of service to the resident." This requirement was not met as evidenced by: Based on interviews, this facility did not retain R1's file after termination of service, which poses an potential health, safety, and/or personal rights risk.

Ensure hazardous items are locked and not unattended

Based on observation, cleaning solutions were left unlocked and accessible in two facility bathrooms and a hallway cabinet, which poses an immediate health, safety or personal rights risk to persons in care.

Based on observation the licensee did not comply with the section cited above in bathroom 2 contained a facet knob in disrepair containing plastic wrapped around it with rubber bands not working and able to use which poses/posed a potential health, safety or personal rights risk to persons in care.

Keep window screens clean and in repair

Based on observation, the licensee did not comply with the section cited above in the window screen in backyard of right of facility in disrepair which poses/posed a potential health, safety or personal rights risk to persons in care.

Keep bath, laundry and kitchen floors clean

Based on observation the licensee did not comply with the section cited above in R1 room contained leaks from the ceiling due to rain on 10-24-21. R1 has been sleeping in the room with the leaks which poses/posed a potential health, safety or personal rights risk to persons in care.

Maintain facility in clean, safe, sanitary condition

Based on observation the licensee did not comply with the section cited above in the kitchen flooring in disrepair and hood of over the stove accumulating grease which poses/posed a potential health, safety or personal rights risk to persons in care.
