What the inspector wrote

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to collateral inspection and met with Administrator, Victor Burachek. LPA stated the reason for the inspection was to interview resident (R1) who recently moved to this facility from another local facility where concerns were made to the Department. LPA spoke to (R1) for about (40) minutes, in their room, about concerns at the prior care facility where (R1) lived. (R1) was able to provide detail on each allegation discussed. (R1) expressed feeling comfortable at Golden Crest Care Home and that they realize they need daily assistance with activities of daily living (ADL's). LPA observed a copy of a current physician's report (LIC602) for (R1) dated, 9/9/25, when (R1) was discharged from the hospital. The LIC602 notes (R1) requires assistance with all ADL's except for eating and is not able to leave the facility unassisted. The LIC602 also notes (R1) can be disoriented and lack hazard awareness, impulse control and expresses frustration occasionally. Also discussed today was Administrator's RCFE Certificate (valid through 9/29/25) was submitted recently for renewal. LPA discussed how an annual inspection will be conducted by 11/30/25. There are no deficiencies issued in this report. Exit interview. Copy of report will be provided by email by close of business today.
