Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a required annual. LPA met with Gabriela Vieru, Administrator, and explained purpose of inspection. Also present was Corine Crooks, caregiver. LPA observed (5) residents present at the start of the inspection and (1) resident return during the inspection. The facility is licensed for (6) non-ambulatory residents and has a hospice waiver for (6). There is (1) resident on hospice at this time. There is pending ownership change. LPA and the Administrator toured the interior/exterior of the facility including the common areas, resident bedrooms (4), resident bathrooms (2.5), kitchen, staff room and laundry area, and observed the facility to be clean, organized, in good repair and odor-free. Bathrooms have the necessary grab bars, non-skid flooring, paper towels and 20-second hand-washing poster. Hot water measured 111*F in a resident bathroom. There is sufficient 2+day perishable and 7+day non-perishable supply of food, and locked sharps in the kitchen. Toxins are secured in the laundry area and medications are secured nearby. The inside temperature measured 79*F. Smoke/monoxide alarms are working and the fire extinguisher was last serviced 1/27/25, and quarterly fire drills are conducted. Paper supplies, including PPE are on hand. There is sufficient space outside, including a covered area for seating and activities. There is (1) unlocked exit gate from the back yard, and an enclosed front and side patio areas where residents can have visitors. LPA reviewed (3) resident and (3) staff files. Files are organized and contain the required documentation. Medications were reviewed for (2) residents- orders match medications being administered. All staff is cleared, has current First Aid/CPR and has completed the required annual training. Administrator RCFE Certificate # 7001957740- (exp 9/18/25) is pending renewal. Required postings are in the common area. The facility has obtained an updated Dementia care plan and updated Admission Agreements pre regulation changes in January 2025. There were no deficiencies observed during today's inspection. There are no citations issued in this report. Exit interview with Administrator. Copy of report provided.
