What the inspector wrote

Furthermore, LPA Martinez conducted an interview with witness 3 (W3). W3 reported the facility has been providing satisfactory care to R1. W3 reported at this time, there are no concerns with the care that is being provided to R1. Due to the above noted information, although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, and therefore the allegations are unsubstantiated. An exit interview was conducted, and a copy of this report was given to the facility at the end of this visit.

This requirement was not met as evidence by: the facility did not have a copy of the home health agency and licensee written care responsibilities agreement. This posed a potential health and safety risk to residents in care.
