Observation of the ResidentThe licensee... that residents are regularly observed for changes in physical, mental, emotional and social functioning and that appropriate assistance is provided...The Licensee did not meet the above requirement as evidenced by: R1 had a bar of soap inside their vagina which was not discovered until R1 was at the hospital. This posed an immediate risk to the health, safety and personal rights of residents in care.
