Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection utilizing the inspection tool. LPA met with staff who contacted Administrator who arrived to the facility shortly afterwards. Today's census is five residents in care with three residents on hospice services. Facility is currently licensed for six non-ambulatory residents with a hospice waiver for four. LPA observed Administrator Ce rtificate #7 007681740 to be active with expiration date of February 23, 2026. Today's inspection, LPA and Administrator conducted a tour of the facility to ensure the health and safety of residents in care. LPA observed facility to have Community Care Licensing and Long Term Care Ombudsman compliance poster to be posted in a conspicuous space. LPA observed residents to be in their private rooms. LPA observed facility to be at a comfortable temperature for residents in care at 73* F. LPA observed carbon monoxide alarms to be in working condition. Fire extinguisher was recently serviced on July 14, 2025. LPA observed residents rooms to be free of medications, toxins, and sharps. LPA observed bathrooms to be clean with non-skid mats and paper towel for resident usage. LPA observed the kitchen to be free of pest and/or rodents, with the presence of at least two days of perishable and seven days of non-perishable food for residents in care. LPA observed food to be kept in a safe manner away from cleaning supplies. File review was conducted. LPA observed all required documents to be complete. Emergency Disaster Plan was recently reviewed on May 15, 2025. LPA observed fire drills documented to be conducted quarterly. Liability insurance was observed to be current and active, compliance to Health and Safety Code. CARE tool was completed and found facility to be in compliance. No deficiencies cited. Exit interview and a copy of the report was provided.
