Follow-up on corrections·March 24, 2026

On 3/24/2026 Licensing Program Analyst (LPAs) Shakaricka Hughes and Pang Lee conducted an unannounced visit to the facility. The purpose of the visit was to clear ensure previously cited deficiencies have been corrected. LPAs met with the licensee to review plans of correction. On March 10, 2026 the facility was cited under Title 22 regulation 87204(a) Limitations- Capacity and Ambulatory Status. LPAs toured the facility and observed the current facility census is (6) residents. LPAs did not observe resident (R1) in the facility at the time of the visit. The licensee stated that the resident was sent to the hospital on March 16, 2026 and will not be returning to the facility. An exit interview was conducted, and a copy of this report and POC letter was generated and provided to the facility.
