Correction check·January 28, 2026

On 1/28/26 Licensing Program Analyst (LPA) Kevin Gould conducted an unannounced Plan of Correction (POC) inspection to ensure previously cited deficiencies have been corrected. LPA conducted a tour of the facility to ensure agreed upon corrections have been completed. LPA conducted an inspection of the facility kitchen and observed new locks on kitchen cabinets and drawers where cleaning supplies or sharp objects are stored. LPA inspected the facility medication storage area and observed a new lock on the door and the medications are now observed to be secured from residents and visitors. LPA also inspected the facility bathroom and observed the toilet with a broken tank cover has been replaced with a new toilet. LPA Gould has cleared all deficiencies and generated the POC letter confirming the corrections. Exit interview conducted and a copy of this report and POC clearance letters were left at the facility.
