on 10/29/2025 by Licensing Program Analyst (LPA) Cynthia Tamayo conducted an unannounced case management visit at this facility. This LPA was met by the facility designated Administrator Zoilo (John) Cendana and Diana Cendana who was briefly interviewed at this time. The purpose of this case management visit was to discuss the Unusual Incident Report (SIR) and SOC 341 that were submitted to Community Care Licensing on 10/30/2025. The SIR were submitted into CCL for an incident of suspected abuse of a resident that was reported on 10/29/2025. LPA was present to discuss the background behind the incidents, follow-up by this facility and it's staff, and resident at that time with. There were no deficiencies cited during today's case management visit. Exit Interview conducted and a copy of this report was left at the facility.
