Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a required annual and met with David Miclea, Administrator. Also present was staff, Mary and Emma LPA observed (2) residents in the common area watching television. The facility is licensed for (6) non-ambulatory residents and has a hospice waiver approved for (2) residents. Currently there are (0) residents on hospice. The facility resumed operations on/around August 2024. LPA and Administrator the interior and exterior of the facility including the common areas, (2) private resident bedrooms (2) shared resident bedroom, (2) resident bathrooms, (1) staff room in the staff area, office, kitchen, laundry/garage area. LPA observed the facility to be clean, in good repair and odor-free. LPA observed the bathrooms to have the necessary grab bars, non-skid flooring. Sharps and medications are locked in the kitchen and toxins are secured in the laundry area. There is sufficient 2+day, including fresh produce and 7+ non-perishable food. There is a sufficient supply of linens/towels/blankets/PPE, and a complete First Aid kit on site. LPA observed all required postings are posted and the fire extinguisher to have been serviced 1/14/25. Smoke/monoxide alarms are in working order. Hot water measured 109*F in the bathroom and the inside temperature measured 71*F. There is an emergency supply kit, automatic night-lights and extra water. There are indoor activities/board games and outdoor activities. There is seating outside with an umbrella. Residents are able to leave the facility in the event of an emergency. LPA reviewed (4) staff and (2) resident files. Files were organized, complete and current. Administrator just obtained an updated Dementia care plan and will update the Admission Agreement to reflect regulation updates in 2025. Staff have completed required training, including First Aid/CPR. Admin certificate is current. All staff is cleared/associated. There are no deficiencies issued. Exit interview. Copy of report provided.
