Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a required annual and met with David Miclea, Administrator. Also present was staff, Mary Minto and Emma Miclea. LPA observed (2) residents in the common area eating breakfast, and (1) resident was resting in their room. The facility is licensed for (6) non-ambulatory residents and has a hospice waiver approved for (2) residents. Currently there are (0) residents on hospice. (1) resident left to an appointment with their family member. LPA and Administrator the interior/exterior of the facility including the common areas, (2) private resident bedrooms (2) shared resident bedroom, (2) resident bathrooms, office, kitchen, and laundry/garage area. Bathrooms have the necessary grab bars, skid-resistant flooring and hygiene supplies. Sharps and medications are locked in the kitchen; toxins are secured nearby and in the laundry area. There is sufficient 2+day, including fresh produce, and 7+ non-perishable food. There are safety locks on the stove, sufficient linens/towels/blankets/PPE, and First Aid supplies. Smoke/monoxide alarms working, and the fire extinguisher was last serviced 1/8/2026, Hot water measured 120.4*F in the kitchen; inside temperature measured 72*F. There is a water fountain near the front entrance that is filtered through rocks with no water collected. There are ramps at the front/back entrances, and each exit door has auditory alerts. Safety drills are conducted. There is an emergency supply kit, generator, automatic night-lights and extra water. All exit doors are free of obstruction and staff are aware of the location of the utility shut-off valves. All required postings are visible in the common area. LPA and Administrator discussed a variety of topics. LPA reviewed (4) staff and (2) resident files. Files were organized and current. Medications were reviewed for (1) resident- no discrepancies noted. Staff have completed required training, including First Aid/CPR. Administrator certificate is current. All staff are cleared/associated. There are no deficiencies issued. Exit interview. Copy of report to be emailed.
