What the inspector wrote

On 8/20/25 at 10:45am, Licensing Program Analyst (LPA) Kevin Gould conducted an unannounced Case Management inspection to address concerns the facility is not following all aspects of an exception for a resident with G-Tube feeding. LPA met with identified administrator Gloria Ramirez. LPA received a written plan of correction indicating the facility was unable to provide an updated care plan to for the exception for the prohibited health condition for Resident R1 (See confidential names list, LIC 811 dated 8/20/25). Currently there is no licensed professional available to provide feedings and ensure injected medications are provided to R1 due to R1's change in condition where they are no longer able to administer own feedings and injections. At the time of this report the The facility is not meeting the stipulations of the exception waiver for the prohibited health condition and could not provide information on how the facility would again be complaint with the stipulations of the exception waiver. Due to the information above the department has revoked the exception for a prohibited health condition for G-Tube with immediate effect. The facility must provide an 30 day eviction notice to the resident and their authorized representatives and provide a copy of the eviction notice to the department by close of business on 8/21/25. Exit interview conducted an a copy of this report was left at the facility.
