Maintain personnel records at facility location

Based on record review administrator’s file was not in the facility for LPA Lee to review which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review S1 did not have current training in S1’s file and first aid/CPR had expired on 07/12/23, which poses/posed a potential health, safety or personal rights risk to persons in care.

Maintain an approved first aid kit on-site

Based on observation and interview the licensee did not comply with the section cited above. The administrator did not ensure that there were a complete first aid kit in the facility which poses an immediate health, safety or personal rights risk to persons in care.

Keep prescriptions in original containers

Based on observation, interview and record review the licensee did not comply with the section cited above. . Records reviewed indicated that resident 1 (R1) Quetiapine 25 MG medication not stored in its originally received container. The instruction for the medication is to take one-half tablet by mouth. It was learned that care staff pre-cut the tablet and stored the half tablets in an extra container and created a label for the Quetiapine medication. which poses an immediate health, safety or personal rights risk to persons in care.

Based on observation and interview LPA Lee reviewed 5 out of 6 resident files and based on record review two resident diagnosed with dementia does not have a current LIC 602 Physician Report in their files.

Maintain facility in clean, safe, sanitary condition

Based on observation and interview, LPA Lee observed in resident bedroom # 3 the sliding screen door was not in good repair. LPA Lee observed care staff Helen tried to put the screen back on the rail. Care staff stated that she was not able to put the screen back on the rail. This posed a potential room risk to residents in care.

Maintain required personnel records for staff

Based on observation and interview LPA Lee also requested to review administrator Joyce Diala’s file, and it was learned that facility staff was not able to be located administrator's file. A brief interview was conducted with administrator Diala, who stated that she is not sure why her file is not at the facility and will follow-up on her file.

Provide resident hot water for personal care

Based on observation of water temperature measurement readings, the licensee did not comply with the section cited above in a facility bathroom, which poses an immediate health, safety or personal rights risk to persons in care.

Arrange appropriate medical and dental care

87465 Incidental Medical and Dental Care (a) A plan for incidental medical and dental care shall be developed...:(1)The licensee shall arrange, or assist in arranging, for medical... care appropriate to the conditions and needs of residents. This requirement was not met as evidenced by: Based on observations, interviews, and record review, the licensee did not obtain a plan for medical care regarding the rashes for 2 out of 3 residents. This poses an immediate health and safety risks to residents in care.

Right to accept or decline care or services

87468.1 Personal Rights of Residents in All Facilities (a) Residents in all residential care facilities for the elderly shall have all of the following personal rights: (16) To receive or reject medical care or other services. This requirement was not met as evidenced by: Based on observations, interview, and record review, residents sustained unidentified rash while in care. Diagnosis of rash was not confirmed until 5 months after onset of rash. This poses an immediate health and safety risk to residents in care.

Maintain facility in clean, safe, sanitary condition

Based on observation, the licensee did not comply with the section cited above in 2 out of 2 common areas, 1 out of 2 bathrooms, and 2 out 4 bedrooms, which poses an immediate health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above in 1 out of 2 staffs did not wear a mask, which poses an immediate health, safety or personal rights risk to persons in care.
