What the inspector wrote

On 3/10/2026, Licensing Program Analyst's (LPA's) E. Conchas and Andrew Martinez made an unannounced visit to the facility to initiate a complaint investigation for complaint numbers 56-AS-20260302095529 and 56-AS-20260302151211. LPA's met with Maria Cervantes and explained the reason for the visit. LPA's observed two (2) staff files with no finger print cleared. A citation was issued. One deficiency was cited during this visit. An exit interview was conducted where this report and appeal rights were discussed and copies were provided to Maria Cervantes, Assistant administrator.
