Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Anna Bueno conducted an unannounced visit to this facility to conduct a complaint investigation on the above allegations. LPA met with staff Makaila Tonan and Lidya Wenny and advised them of the purpose of today's visit. The investigation consisted of interviews and review of pertinent documents. The allegation states that the facility air conditioner (AC) is in disrepair. Based on document reviews and interviews, LPA was able to corroborate the allegation. LPA viewed documents confirming the facility was notified of the broken AC on 7/24/21 and an order for AC compressor dated 8/9/21. Interviews confirmed that the AC was repaired on 9/8/21. Based on today's investigation, the above allegation is SUBSTANTIATED. A finding that the complaint is SUBSTANTIATED means that the allegation is valid because the preponderance of the evidence standard has been met. An exit interview was conducted with and a copy of this report was provided to Tonan.
