Based on LPA record review, the licensee did not comply with the section cited above by staff missed administering three (3) of resident #1's (R1's) morning medications; which poses an immediate health, safety or personal rights risk to persons in care.

Obtain required California clearance or exemption

Based on LPA record review and observations, the licensee did not comply with the section cited above by staff#1(S1) documented as a caregiver and residing at the facility without a criminal record clearance or exemption; which poses an immediate health, safety or personal rights risk to persons in care.

Health screening and fitness requirements

Based on LPA record review, the licensee did not comply with the section cited above by the Administrator, Staff# 2 (S2), Staff#3 (S3) did not have a health screening on file which indicates they are physically qualified to perform their job duties and signed by examining physician; which poses/posed a potential health, safety or personal rights risk to persons in care.

87465 Incidental Medical and Dental Care(b) If the resident's physician has stated in writing that the resident is able to determine and communicate his/her need...(2) Once ordered by the physician the medication is given according to the physician's directions. This requirement is not met as evidenced by: Based on interview and record review, the Administrator did not ensure that the resident(s) in care were given their medications according to the physician's orders which posed/poses a potential risk to the health and safety of residents in care.

87303 Maintenance and Operation (a) The facility shall be clean, safe, sanitary and in good repair at all times. Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors.

Minimum appraisal documentation requirements

Based on LPA file review, the licensee did not comply with the section cited above by not maintaining a complete appraisal on file for resident #1 and resident #2, which poses/posed a potential health, safety or personal rights risk to persons in care.
