What the inspector wrote

L icensing Program Analyst (LPA) Magda Malcore made an unannounced visit to the facility to conduct a required comprehensive annual inspection. LPA met with Chelsea Plank, Business Office Director, and discussed the purpose of the visit. The facility is Residential Care Facility for Elderly (RCFE) with a license capacity of (116), and a current census of (103). LPA conducted a general inspection of facility, which included, but was not limited to, the following: Operation/Physical Plant: Indoor and outdoor passageways were kept free of obstruction. The facility has no swimming pool or similar bodies of water. The facility has sufficient activity supplies and space for resident activities and visitors. Activities areas were maintained clean. Outdoor activity space is shaded and protected from traffic. Ten (10) resident bedrooms were furnished with mattresses, bed linen, chairs, night stands, storage space, and lighting. Resident bathroom equipment were operating in safe and sanitary conditions. Bathrooms were equipped with non-skid mats and grab rails. The hot water in resident bathrooms measured 109 degrees (F). The facility is equipped with operating smoke detectors/carbon monoxide alarms, fully charged fire extinguishers, elevator, laundry rooms/equipment, covered fireplaces, and telephone service. Resident personal rights, Community Care Licensing complaint poster, Ombudsman poster, evacuation sketch, facility license, planned activities, menus, "oxygen in use" signs were posted in appropriate areas. Chemicals and cleaning supplies where maintained in a locked storage room. Centrally stored medications and records are kept locked in a medication room. **continued on next page*** Food Service: Kitchen and dining areas in both the memory care wing and assisting living wings were maintained cleaned. Kitchen ventilation system was operating properly. Non-perishable and perishable food supply is sufficient for number of residents in care. The facility has a dedicated Special/modified diet list which was observed in the kitchen area. Care & Supervision: The facility has care staff coverage, 24 hours a day, 7 days a week. All staff working in the facility have criminal record clearance through the Department. Record Review: Ten (10) resident files were reviewed for admission agreements, physicians/medical assessments reports, appraisals, needs and services plans. Ten (10) staff files were reviewed for First Aid/CPR certifications, criminal record clearances, job related training, and health screenings with tuberculosis results. The Executive Director’s administrator certification is current. The facility maintains an infection control plan, emergency/disaster plan, liability insurance, client registry and staff disaster training at the facility. No deficiencies were cited during today's visit. An exit interview was conducted where this report was discussed and a copy provided to the Business Office Director at the conclusion of the visit.
