What the inspector wrote

Six (6) of six (6) residents interviewed report insects in their bedroom or bathroom. Six (6) of six (6) residents interviewed report that they have not seen any rodents in the facility. LPA tour of the kitchen and food storage areas did not reveal that the kitchen was in disrepair. Based on the information available the alleged violation that the facility has insects has been substantiated as valid and that a violation has occurred based on the preponderance of available evidence. A copy of this report along with appeal rights are being reviewed with, and furnished to the facility representative. Please see LIC 9099D. Six (6) of six (6) residents interviewed report insects in their bedroom or bathroom. Six (6) of six (6) residents interviewed report that they have not seen any rodents in the facility. LPA tour of the kitchen and food storage areas did not reveal that the kitchen was in disrepair. It is alleged that the kitchen is in disrepair and that the facility has rodents. Based on the information available, we have found the complaint allegations unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. A copy of this report is being reviewed with and furnished to the facility representative.
