What the inspector wrote

Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced visit to the facility to initiate a complaint investigation. LPA met with Assisting Administrator, Nick Hamed, who was informed of today’s visit. Regarding the allegation staff intimidating and verbally threatening a resident in care. Five (5) out of (5) residents deny that staff intimidate and verbally threaten them. Four (4) out of (4) staff deny intimidating and verbally threatening residents in care. Based on interviews with residents and staff, the allegation above is Unsubstantiated; meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. An exit interview was conducted with Administrator Hamed and a copy of this report with appeal rights was provided to Administrator Hamed at the conclusion of the visit.
