What the inspector wrote

On 11/14/2025 at 1:08 PM, Licensing Program Analyst (LPA) LaVette Farlow conducted an unannounced case management visit regarding a incident of resident on resident abuse. LPA Farlow met with and was granted entry into the facility by Caregiver Galo Jarlego. LPA disclosed the purpose of the visit, review residents files, received documentation, and toured the facility. On October 2, 2025 at approximately, 10:35 PM resident one (R1) returned to the facility intoxicated from being in the community. R1's behavior was aggressive and verbally arguing with his roommate resident two (R2). When staff one (S1) attempted to intervene and de-escalate the incident R1 attempted to hit S1 with his walking cane. S1 stated R1 aggressive behavior continued and Law Enforcement was called, resulting in R1's arrest. S1 provided a police report from the Rialto Police Department, #932-511-7385. At 2:00 PM, LPA observed staff records and found that S1 completed a incident report and started the eviction process for R1. Based LPA's interviews, and records review it was revealed that the administrator did not submit a SIR to CCL as required by Title 22, Division 6, Chapter 8. Administrator and House Manager acknowledge the SIR are complete but have not submitted them San Bernardino Regional Office. A deficiency was cited. An exit interview was conducted and a copy of this report, LIC809, LIC809D, and appeal rights were provided to the House Manager, Malik Salem.
