What the inspector wrote

On 5/5/2026, Licensing Program Analyst E. Conchas Licensing conducted an unannounced visit to this facility to follow up on Unusual Incident Report occurring on 4/20/2026. LPA meet with Executive Director, Rachelle Wheaton. Unusual incident report was that a care staff, during incontinence care observed R1 with an intact glove in her diaper. LPA conducted interview with Rachelle Wheaton regarding incident and conducted a health and safety check with resident involved. R1 was observed sleeping in room. No health concerns were observed at the time.
