What the inspector wrote

Based on LPA observations, interviews which were conducted and records review, the preponderance of evidence standard has been met. Therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations Title 22 are being cited on the attached LIC 9099D. This report was discussed with Executive Director Enriquez, as well as the appeal rights and a signed copy was left with the facility.
