What the inspector wrote

Licensing Program Analyst Paola Guerrero, (LPA) arrived at Oakmont of Chino Hills, unannounced to conduct a Case Management Visit for a follow-up visit concerning bedbugs. This case management visit is in response to a Special Incident Reports, (SIR's) submitted to the Community Care Licensing Office on 04/16/2026 & 4/20/2026. LPA was greeted by Facility Administrator Janeth Medrano. LPA explained to Janeth the purpose of the visit and informed Janeth that a health and safety check will be conducted. LPA explained to Janeth that documents of bedbug treatment and treatment plan will be reviewed and collected by the LPA. ***Health and Safety Check*** Licensing Program Analysts (LPA) Paola Guerrero conducted an unannounced visit to the facility on 04/29/2026 at 09:40 AM for the purpose of a Health & Safety check. LPA Guerrero identified herself with Janeth Medrano and discussed the purpose of the visit. Residents in care were present during visit. No imminent health and/or safety concerns observed at the time of visit. During the visit LPA inspected Room #308A, 214, 335, LPA observed that as of 4/29/2026 no new cases of bedbugs have been reported. LPA collected invoices and continued the inspection. During the inspection LPA Guerrero observed no health and/or safety hazards inside the facility. The needs of the residents in care appear to be met during this inspection. No deficiencies were observed during this visit. An exit interview was conducted where this report was reviewed, discussed and then provided to Administrator Janeth Medrano.
