What the inspector wrote

Licensing Program Analyst (LPA) Magda Malcore made an unannounced visit to the facility to conduct a required comprehensive annual inspection. LPA met with Administrator Carol Kanani Hamilton, was granted entry to the facility, and discussed the purpose for the visit. The facility is a Residential Care Facility for Elderly (RCFE) with a license capacity of (4), and a current census of (4). LPA conducted a general inspection of facility, which included, but was not limited to, the following: Operation/Physical Plant: The facility is operating in the capacity approved by Community Care Licensing Division (CCLD). Indoor and outdoor passageways were kept free of obstruction. Outdoor pool area is fenced. Outdoor activity area is shaded and gated. The facility's living room, family room, and dining areas are sufficient for resident activities. The facility is maintained at a comfortable temperature of 73 degrees Fahrenheit. Resident bedrooms were furnished with beds, night stands, chairs, bed linen and sufficient lighting. Resident bathroom was maintained clean and fixtures were operating properly. The hot water temperature in the bathroom measured 116 degrees F. The facility is equipped with operating smoke detector/carbon monoxide alarms, fully charged fire extinguisher, laundry equipment, covered fireplace and telephone service. The facility has posted in a common area Community Care Licensing complaint poster, Ombudsman poster, facility license, facility sketch and emergency telephone numbers. The facility has 24 hour/7 days a week staff coverage. LPA observed disinfectants sprays and cleaning supplies stored unlocked and unattended in facility garage. Administrator removed the chemicals and placed in them in a locked cabinet. Food Service: Kitchen and dining areas were maintained cleaned. Non-perishable and perishable food supply was sufficient for number of residents in care. Cups, plates, and utensils were sufficient for number of residents in care. The facility’s refrigerators and freezerz were operating properly. Health Related Services: The facility maintains record of resident’s medications and medications were centrally stored in a locked cabinet. Record Review: Three (3) resident files reviewed had admissions agreements, physician’s reports, appraisals, needs and services plans. Review of three (3) staff files reveals staff #1 (S1) did not have record of yearly Dementia training for review. Administrator stated that training was conducted; however was not sure of the date. The Administrator’s certification, facility’s insurance and emergency drill training are up to date. Deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted where reports (LIC809/LIC809D) and correction plans were discussed. Report copies were provided to Administrator Hamiliton with appeal rights at the conclusion of the visit.
