What the inspector wrote

Licensing Program Analyst (LPA) Michelle Echeverria conducted an unannounced visit to this facility to discuss a death report that was received by the Department on 06/17/25. LPA met with Staff, Bria Sheriff. During today's visit, LPA performed a health and safety check, and reviewed records. No deficiencies were cited during today's visit. An exit interview was conducted where a copy of this report was provided to Licensee, Angela Breedlove who later arrived.
