What the inspector wrote

On 11/13/2025 at 3:30PM Licensing Program Analyst (LPA) Renese Howell-Small conducted an unannounced visit to the facility in order to conduct a Case Management - Deficiencies visit. LPA met with Resident Services Director, Dulce Allen. LPA conducted a tour of the memory care unit and observed that the "planned social and recreational programs" were limited. LPA observed the residents to be watching television and a few residents walking in the outdoor courtyard. LPA did not observe any activities provided for interaction and cognitive stimulation. A deficiency will be cited. An exit interview was conducted where this report LIC809, LIC809D and Appeal Rights were discussed and copies provided to the Resident Services Director.
