What the inspector wrote

On 03/02/2026 at 3:00PM Licensing Program Analyst (LPA) Renese Howell-Small arrived unannounced to the facility in order to conduct a Case Management visit. LPA met with Executive Director, Ruth Villa and stated the purpose of the visit. LPA toured both the assisted living and memory care units. LPA observed several resident bathrooms in the memory care unit to be unsanitary. A deficiency will be cited. An exit interview was conducted where this report LIC809, LIC809D and Appeal Rights were discussed and copies were provided to Community Liason Director, Crystal Wilkins.
