What the inspector wrote

Licensing Program Analyst (LPA) Magda Malcore conducted an announced visit to conduct the Pre-licensing inspection. LPA met with Corporate Representative, Nitin "Nick" Vermani, and discussed the purpose of the visit. The application is for a Residential Care Facility for the Elderly (RCFE). A fire clearance was granted by San Bernardino County Fire Department for a total capacity of six (6) residents (four (4) residents may be non-ambulatory). LPA toured the interior and exterior areas of the facility. The following were inspected: Physical Plant: The physical plant is consistent with floor plan submitted by the facility. Indoor and outdoor passageways were kept free of obstructions. The facility has no swimming pools or similar bodies of water. No firearms are stored at the facility. The facility has hallway night lights, fire alarm/carbon monoxide alarms, fire extinguisher, laundry equipment and door signal system. The facility has bed linen and personal hygiene supplies. Resident bedrooms: Resident four (4) bedrooms were equipped with clean mattresses, pillows, nightstands, and lighting. Resident bathrooms : Resident three (3) bathrooms were equipped with grab bars, and non-skid materials. Food Service/Dining Areas: Kitchen and dining areas were kept clean and odor free. The refrigerator temperature was maintained at 40 degrees Fahrenheit and freezer was maintained at 0 degree F. Facility has sufficient food storage space. Medication: The facility has a location were medication will be centrally stored and kept locked. The facility has a complete first aid kit with manual. Activities : The facility has sufficient living room and indoor patio space for resident activities. Outdoor activity area is gated and has sufficient shaded space for residents. Postings: The facility has posted in the entry area, emergency exit plans, emergency telephone numbers, Community Care Licensing Complaint Division poster (CCLD), Resident Personal Rights, and Resident Council Rights. Comp III orientation was completed during today's inspection. The following Corrections needs to be made prior to licensing: Hot water temperatures in three (3) resident's bathroom did not attain a temperature of not less than 105 degree F (41 degrees C) and not more than 120 degree F (49 degrees C) . Hot water temperatures in bathrooms tested at 65 degrees Fahrenheit. Corporate Representative Nitin stated that the correction will be completed today, 10/30/2025. Corporate Representative Nitin was informed to send proof of correction to the Licensing Agency. An exit interview was conducted where this report was discussed and a copy of this report was provided to Corporate Representative Nitin the conclusion of the visit .
